本研究は,周手術期の肺がん患者への身体的・心理的にサポートするための術前オリエンテーションプログラム(以下,プログラム)を実施,評価することを目的とする。同意を得られた初めて手術を受ける肺がん患者10名(男性6名,女性4名),平均年齢62.8歳を対象とした。作成したパンフレットとDVD(22分)を用いたプログラムを,外来通院中で手術の説明を受けた後の期間に個別に施行後,手術に対する認識の変化について半構成的質問紙を用いた面接とプログラムの評価についてアンケートを行った。その結果,対象者の認識はプログラム参加前には「ショック」「死の予感」など,プログラム終了後には「具体的な理解」「前向きな態度」「痛みを我慢しない」などを抽出した。プログラムの評価は,ほとんどの患者からよくわかったという評価を得た。プログラム終了後に前向きな認識の変化により,プログラムに参加することで患者は,手術への準備を具体的に知り術後回復に前向きに取り組むための情緒的サポートを得たと考えた。 |

The present study was undertaken to prepare, implement, and assess a pre-operative orientation program designed to provide physical and mental support to lung cancer patients during the perioperative period. The study involved 10 outpatients with lung cancer (6 males and 4 females, with a mean age of 62.8 years) who were scheduled to undergo first surgery and who gave informed consent to participate in the study. The orientation program was implemented using a leaflet and DVD (22 minutes). After the orientation program, each subject was interviewed with a semi-structured questionnaire to check for changes in their understanding of the planned surgery. The program was evaluated by a questionnaire. Before orientation program, patient awareness was characterized by "shock," "possibility of death," and similar reactions. After orientation program, it was characterized by "firm understanding," "positive attitude towards surgery," "avoiding bearing the pain with stoical resignation," and similar attitudes. Most patients assessed the orientation program as easy to understand. The positive changes observed in patient awareness after the orientation program suggest that patients who participate in it obtain concrete information needed for them to become prepared for surgery and gain emotional support needed to facilitate smooth postoperative recovery. |
